How to get started with Willow, what to expect from your free assessment, and common questions about the program

Written by Eliza Snow

At Willow, we believe weight loss care should be personal. Our doctors take the time to understand your body, your goals, and your health history, then match you with a treatment plan tailored to you - no office visits, no waiting rooms. Getting started takes about 15 minutes, and your doctor can approve your treatment the same day.

Three steps to get started

Step 1: Free assessment

Visit startwillow.com and click “Start My Free Visit.” You’ll answer a short series of questions about your health history, current medications, and goals. This takes most people around 10–15 minutes. The assessment is free. Your information is kept private and used only for your medical review.

Step 2: Choose your treatment option

If you’re eligible, you’ll select your preferred treatment option. A U.S.-licensed Willow doctor will review your information and confirm your prescription, often the same day. Your doctor creates a personalized treatment plan based on your health profile.

Step 3: Move forward with confidence

Your medication ships directly to your door with free, discreet delivery. From there, your Willow Care Team is with you every step of the way, with doctor messaging, free follow-ups, video guides, and access to a peer community of people on the same journey.

Willow is FSA & HSA eligible. If you’d like to use your FSA or HSA card toward your program, learn more here.

What’s included in your program

Your Willow membership includes everything you need to manage your treatment:

  • Personalized treatment plan created by a U.S.-licensed doctor based on your individual health profile

  • Custom-formulated GLP-1 medication prepared by trusted U.S. pharmacy partners, with free discreet delivery to your door each month*

  • Injection supplies are included with each shipment.

  • Ongoing doctor access for questions, prescription updates, and dose adjustments through the patient portal

  • Free regular follow-ups from your Willow Care Team

  • Video guides and peer community access throughout your treatment

*Willow’s GLP-1 medications are compounded by licensed U.S. pharmacy partners using ingredients commonly prescribed in medical practice. Willow is not the manufacturer. Compounded medications are not FDA-approved, but are prepared under applicable pharmacy regulations.

Am I eligible?

Your eligibility is determined by a Willow doctor during your intake review, not by an algorithm. To be considered for the program, you generally need to:

  • Be 18 years of age or older.

  • Reside in a state where Willow currently operates.

  • Have a BMI that meets the clinical threshold for GLP-1 treatment (reviewed during your intake)

  • Not pregnant, breastfeeding, or planning to become pregnant

  • Pre-existing conditions such as hypertension or high cholesterol do not automatically disqualify you. Your doctor reviews your full health history before making any decision.

Common questions

Is the assessment really free?

Yes. Your intake assessment and doctor review cost nothing. You only pay if you’re approved and choose to move forward with treatment.

How much does Willow cost?

Willow’s program starts at $299/month, which includes your medication, doctor oversight, supplies, and ongoing support. There are no hidden fees.

See How much do treatments cost? for full pricing details.

What if I’m not approved?

If a Willow doctor determines that GLP-1 treatment isn’t right for you at this time, you won’t be charged. Your doctor may explain why and suggest alternative options.

Does Willow accept insurance?

Willow does not currently bill insurance directly. FSA and HSA funds can often be applied toward your program.

Can I cancel?
